The application may be found here: https://docs.google.com/forms/d/e/1FAIpQLSdI0KhkkiHZhXEQMI9sSXFHM4zIVcLf8WIKBa960fqfijYdsQ/viewform. You will need to upload your essays and resume (as one combined file) directly via the Google Form when you submit your personal information (instead of e-mailing the documents).

STEP 1: Personal Statements

ESSAY #1:
Describe the world you come from and how it has shaped who you are (500 words or less)

ESSAY #2:
Describe an example of your leadership experience in which you have positively influenced others, helped resolve disputes, or contributed to group efforts over time. (350 words or less)
**This similar to UC PIQ #1. Feel free to use or modify your response.

ESSAY #3:
Describe the most significant challenge you have faced and the steps you have taken to overcome this challenge. How has this challenge affected your academic achievement? (350 words or less)
**This is the same as UC PIQ #5. Feel free to use or modify your response.

STEP 2: RESUME

Create a resume that includes a list of extra-curricular and community service activities that you have participated in (including the name of the activity, your role, and the dates that you participated).
STEP 3: Contact information

Fill out the application by clicking "Continue" at the bottom of this page and enter your contact information, high school name, weighted GPA, college application information, and other details.
STEP 4: Upload

IMPORTANT: Upload your essays and resume on this form as ONE COMBINED Word or PDF documents in the following format: Lastname_Firstname.pdf. Be sure to type your name and the title of each document at the top of the page. ****DO NOT SUBMIT YOUR DOCUMENTS AS SEPARATE FILES. COMBINE AND SAVE THEM AS ONE****
I highly recommend you do not submit your essays without having a Teacher or a Counselor review your responses. You might think it is a good essay and it might not be. Please do not just copy and paste your UC or any College essay. Review your response, edit it, and make sure it is the best essay you have written.

NOTE:

****If you are having any issues with uploading your files, you may e-mail them instead to [email protected]****

****Please do not fill out your application until you have written your essays and resume and are ready to upload them. Applications submitted after February 28th, 2020 will not be considered.****

****This is the first step of a multi-round process. Finalists will notified by April 3rd. In person interviews with finalists will be conducted on April 18th, 2020. ****

The application is due on Friday, February 28, 2020 by 5pm.
